About Lōkahi

Rooted in Hawaiʻi

I’m Lani Hubbard, a board-certified Gerontological and Advanced Certified Hospice and Palliative Nurse Practitioner with more than twenty-five years of experience caring for older adults and their families.

Born and raised in Hawaiʻi, I am a proud graduate of Kamehameha Schools. I earned a Bachelor of Arts in Psychology from Dartmouth College and a Master of Science in Gerontological Nursing from Boston College.

My work has included long-term care, home-based primary care, hospice, and palliative care. Across every setting, I have seen that the best care is not only clinically thoughtful, it is personal, unhurried, and grounded in listening to what matters most to each person and family.

Why I Created Lōkahi

Every older adult has a story. Listening to those stories has shaped my life. Some of the most important lessons I've learned about healthcare didn't begin in a hospital. They began around my grandmother Rose's kitchen table.

After graduating from college, I moved in with my grandmother just outside of Boston. During that time, I worked in a nursing home and assisted living community while preparing to attend graduate school.

What I thought would be a temporary season of life became the beginning of my life's work.

The Kitchen Table

Living with my grandmother gave me the opportunity to experience aging not as a healthcare provider, but as a granddaughter. I watched her health change over time, but I also came to appreciate something even more meaningful—her story.

She shared memories of family, love, hardship, resilience, and the experiences that had shaped the remarkable woman she became. She taught me how to cook, how to bake, and, perhaps most importantly, how much wisdom can be found when we simply slow down and listen.

Working with older adults each day reinforced that lesson. Every life reflected decades of experiences, relationships, accomplishments, challenges, and resilience.

I realized that what I loved most wasn't simply providing care. It was getting to know the person behind the medical record.

Turning Lessons Into a Career

That realization led me to pursue my nurse practitioner education at Boston College, where I began turning the lessons I had learned around my grandmother's kitchen table into the clinical knowledge and skills that would guide my career.

Today, more than twenty-five years later, I still feel that same sense of purpose. Throughout my career, I've had the privilege of caring for older adults and their families in long-term care, home-based primary care, hospice, and palliative care.

Those experiences have reinforced something I believe deeply: Every healthcare decision begins with understanding what matters most.

Clinical knowledge is essential, but so are presence, compassion, curiosity, and the willingness to understand what matters most to each individual and family.

Why Lōkahi Was Created

Over the years, families have often told me that I bring a calm presence during difficult times. I consider that one of the greatest compliments I have ever received.

People deserve more than excellent medical care. They deserve to feel heard, respected, and supported.

As healthcare became increasingly fast-paced and fragmented, I found myself longing for something different—more time to listen, build lasting relationships, support family caregivers, and help people make thoughtful decisions before moments of crisis.

That desire became the foundation for Lōkahi Integrative Health.
